Registered Medical Marijuana Collective Application Forms

Individual Application


 

The City is NOT accepting applications for any new medical marijuana collectives or other medical marijuana businesses, including independent cultivation or manufacturing companies. As of October 17, 2014, the City of San Jose's application period for registration as a medical marijuana collective has closed.

Registered Medical Marijuana Collective Application Forms

The application forms are for Registered Collectives to update or amend their registration only. A collective may not begin operations at the new location or premises until it receives a new Notice of Completed Registration for its new location or premises. The process and the fees for re-registration shall be the same as the process and fees set forth for registration in Section 6.88.330.

When the applications are complete, print the form(s), sign where needed, and prepare all required attachments for submission to the Division of Medical Marijuana Control.

Applications are accepted on Tuesdays and Thursdays by appointment only.

Collective Cultivation / Dispensing Location Amendments

Add / Change Cultivation Site Within San Jose

Any time a collective adds or changes the cultivation site within San Jose, it is required to submit the following:

Any time a collective changes its cultivation site outside of San Jose are required to submit the following:

Collective Delivery Service Applications

Registered Collectives wanting to deliver to registered patients are required to submit the following:

All Registered Collectives updating their Registration Application are required to submit the updated section of the application

Renew Notice of Complete Registration

All Collectives are required to submit a new Application for Registration sixty days prior to the expiration date of their current registration.


Collective Employee Applications to Work at a Registered Collective

Prior to submitting this application, please contact the San Jose Police Department Division of Medical Marijuana Control at dmmc.appt@sanjoseca.gov or call (408) 537-1420 to schedule an appointment for fingerprinting, badging, and payment of associated fees.

 

Collective Employee Application

All individuals participating in the cultivation, processing, manufacturing, transporting or dispensing of medical marijuana are required to complete pages 1-7 of the Employee application. A Collective Manager MUST sign the application PRIOR to your appointment to be fingerprinted.

NOTE: Once you complete the application, submit an email request to schedule an appointment for a background and fingerprint appointment to dmmc.appt@sanjoseca.gov Bring your manager-signed application and all supporting documentation to your appointment.

 

Collective Manager Application

All managers and owners of a Registered Collective are required to complete pages 1 and 2 of the Owner/ Manager Supplemental Application.

NOTE: If you are a new employee also applying as a manager, complete the New Manager Application in addition to pages 1-7 of the Employee application.

 

Delivery Driver Application

All current collective employees applying as a delivery driver are required to submit their Delivery Driver application to the Division. The individual may NOT conduct deliveries on behalf of the Collective until they are approved by the Chief of Police and provided a Delivery Driver Identification Badge

NOTE: If you are a new employee also applying as a delivery driver, complete the Delivery Driver Application in addition to pages 1-7 of the Employee Application.

Only members of a Registered Collective may apply to conduct deliveries on behalf of the Registered Collective pursuant to SJMC 6.88.445.
