Online Police Report FAQ (Frequently Asked Questions)

If you want to provide a crime tip, visit the Crime Stoppers website at www.crimestoppers.com.

The Citizens’ Online Police Report System allows you to submit a police report online and print a copy for your records. Please answer the following questions. Your answers will determine whether filing a report online is right for you:

Yes	No	Is this an emergency? (If yes, please call 911 immediately) Your answer indicates that this is an emergency. Emergencies should be reported by calling 911.
Yes	No	Are you reporting a lost or stolen firearm? We do not take lost or stolen firearm reports on our online service. To file a lost or stolen firearm report call (408) 277-8900
Yes	No	Is there physical evidence of this crime such as video or digital evidence? Your answer indicates that there may be physical evidence such as video or digital evidence. If so, please call (408) 277-8900 to file a report.
Yes	No	Do you believe you are the victim of a crime because of your race, religion, sexual orientation, handicap, ethnicity or national origin? Your answer indicates that you may be the victim of a Hate Crime. If so, please call (408) 277-8900 to file a report.
Yes	No	Did the incident occur on a State highway or freeway? Your answer indicates that the incident occurred on a State highway or freeway. If so please contact the California Highway Patrol Office nearest you, http://www.chp.ca.gov
If you answered NO to all the questions in this section, please continue. Note: Filing a false police report is a crime! Every person who knowingly makes a false report is guilty of a misdemeanor per 148.5 (a) of the California Penal Code.
Yes	No	Do you understand that filing a false police report is a crime? Your answer indicates that you do not understand that filing a false police report is a crime.
Yes	No	Did the incident occur within the San Jose city limits? Your answer indicates that the incident did not occur within the San Jose city limits. If so, please contact the city or county where the incident occurred to file a police report.
Yes	No	Are you eighteen (18) years of age or older? Your answer indicates that you are not eighteen (18) years of age or older. If so, you cannot submit a police report online. Please have a parent or guardian submit the police report for you.
Yes	No	Do you have an email address? (A valid return email address is required in order to file an online report.) Your answer indicates that you do not have an email address. A valid return email address is required in order to provide you with a temporary police report number and a copy of the police report for your records.
